Logging on to My Care Card

Complete the following steps to log on to My Care Card:

1.   From the My Care Card URL, click the Administrator Log on button. The corresponding Logon page is displayed.

2.   In the User ID field, enter your assigned user ID.

3.   In the Password field, enter your password. If you have forgotten your password, My Care Card can help you recover it.

Note: The password field is case sensitive.

4.   Click Log on. One of the following occurs:

·       Your log on is successful.

·       Your log on is challenged and you must verify your identity.

·       If this is your first logon attempt, the Additional Security Information page is displayed.

Note: If your log on attempt was unsuccessful, repeat steps 1 through 3. My Care Card locks your user ID after three consecutive unsuccessful log on attempts. When your user ID is locked, you must contact an administrator to have your account unlocked.
